Applying to Become a Starlight Family
To register as a family with Starlight Children’s Foundation Canada, your child must biologically be under the age of 18 and be diagnosed with a serious illness or permanent disability by a Canadian Medical Practitioner.
While we recognize the difficulties a family may face, the following are some of the examples of a child’s primary diagnosis that we DO NOT accept into our programs: Autism Spectrum Disorder, ADD, ADHD, Down Syndrome, Language Delays, Learning Disabilities and Psychiatric Disorders. Please DO NOT submit an application if your child has one of these as their primary diagnosis. Thank you for your understanding.

Have your child’s medical professional fill out the form below, scan it and upload it when you fill out the Family Application Form.

Step 2:
Once you have the Medical Professional Form filled out (make sure you have an electronic version – picture or scanned pdf), click on the link below to fill out the Family Form.
Without the Medical Professional Form filled out, you will not be able to fill out the Family Application Form.
